1. Eligibility and Professional Use Only

The Site and products (including Ocular Amnion amniotic membrane discs) are intended exclusively for qualified eye care professionals (e.g., licensed ophthalmologists, optometrists, or eye care centers) with a valid National Provider Identifier (NPI).

You represent and warrant that you are a licensed healthcare professional or authorized representative of a professional entity, and that any NPI or other information you provide is accurate and current.

We reserve the right to verify credentials (including NPI via public registries) and refuse or cancel access/orders if verification fails or if use appears non-professional.

The Site is not directed to consumers/patients or the general public. Products are not for self-administration or direct-to-consumer sale.
2. No Medical Advice

All information on the Site (including product descriptions, FAQs, resources, or communications) is for informational and educational purposes only.

It does not constitute medical advice, diagnosis, treatment recommendations, or a doctor-patient relationship.

Always consult the product's Instructions for Use (IFU), full labeling, and your professional clinical judgment for patient-specific application, risks, benefits, and care.

3. FDA Clearance and Promotional Compliance

Ocular Amnion products are FDA-cleared Class II medical devices for specific ophthalmic indications as stated in official labeling.

Clearance does not imply FDA approval or that the product is risk-free or guaranteed effective for all uses.

Any claims on the Site are consistent with cleared indications. We do not promote off-label uses. For full indications, contraindications, warnings, precautions, and adverse events, refer to the product IFU and labeling.
4. Accounts and NPI Verification

To purchase or access certain features, you must create an account and provide accurate information, including a valid NPI.

You are responsible for maintaining the confidentiality of your account credentials.

We may suspend or terminate accounts for violations, inaccurate information, or suspected misuse.
5. Orders, Payments, and Shipping

All orders are subject to acceptance by us. We may refuse or cancel orders (e.g., due to pricing errors, stock issues, or verification failure).

Prices are in USD and exclude taxes/shipping unless stated. Payment is due at checkout via accepted methods.
